Introduction: Brazil exhibits heterogeneous mortality patterns for pulmonary embolism (PE), recording 115,923 deaths between 2001 and 2021. Objective: To analyze the spatial distribution and epidemiological profile related to mortality from PE in Brazil during this period. Methods: This is an epidemiological study with a spatiotemporal analysis using data from the Mortality Information System (SIM/DATASUS) and IBGE. Crude and standardized mortality rates were calculated for Brazilian municipalities. The epidemiological profile included sex, age, race, marital status, and education. Temporal and spatial analyses were performed using Joinpoint and GeoDa software. Results: The highest mortality profile consisted of women (57.49%), individuals aged over 80 years (32.42%), white ethnicity (62.08%), and married individuals (34.24%). Spatial analysis identified high-mortality clusters in the South and Southeast regions. The municipalities with the highest rates were Dom Pedrito-RS (22.64/100k), Balneário Rincão-SC (21.17/100k), and Sant’Ana do Livramento-RS (19.51/100k). The average mortality rate was 2.79/100,000 inhabitants, with a significant average annual increase of 2.25% (p<0.05). Conclusion: PE mortality is increasing in Brazil. Identifying critical areas and vulnerable profiles highlights the need for specific regional strategies and the strengthening of Primary Health Care for the adequate management of this condition.
